We bring education, resources, and services out into our wider community. Our team of dedicated community health educators brings events and programs to the communities we serve. They also connect community members with our clinics.

Mammograms & Breast Health
Mammograms are x-rays that screen for breast cancer early, in its most treatable stages. For women over age 40, annual mammograms are important to staying healthy and cancer-free. We partner with the Minnesota Department of Health Sage Screening Program and Park Nicollet’s Mammo-A-Go-Go mobile mammography unit to host mammogram events at our clinics. The third Wednesday of every month, we provide free or low-cost mammograms. Patients can also learn about cancer prevention and breast health, ask questions, and enjoy food and beverages. Testing for cervical cancer and the common virus that can lead to it (Human Papillomavirus, or HPV) should start at age 21. For those without insurance, we partner with MDH Sage Screening Programs to offer free cervical exams and Pap smears. HPV vaccinations are available for adolescent and adult patients. Did you know that the most common symptom of a sexually transmitted infection (STI) is no symptom at all? For people who have sex, it’s important to get tested regularly, regardless of gender, sexual orientation, or age. Keeping you and your partner(s) safe is as easy as stopping by the clinic. We provide free or low-cost STI testing at all our clinics and at events in the community. We screen for common STIs such as gonorrhea, chlamydia, and HIV. Our community health educators offer confidential, one-on-one conversations with patients about sex, birth control, and STIs. Free condoms are always available in our clinics. Reproductive Health
We have reproductive health services available for everyone. Our providers and community health educators can help you learn about reproductive health and decide which birth control method is right for you. They can also offer referrals to reproductive health resources in your community, and provide ongoing follow-up care. One-on-one sexual health counseling is available at all four of our clinics.

The community health team presents information about clinical services, insurance, cancer screening, chronic disease management, and sexual health at locations throughout the metro area. We provide STI screening, blood pressure checks, appointment scheduling, and sexual health presentations at schools, community centers, places of worship, businesses, and social services organizations. We have a Peer Recovery Coach on staff who works with patients one on one to guide them through the addiction recovery process. A Recovery Coach can connect patients with community resources, remove barriers to recovery, and be a needed support during this challenging process.
Our Community Health Team runs several programs to engage youth in topics such has health and peer health education, violence prevention, and community healing. Learn more about all our youth programs here.
Pop-Up Clinics
Our providers and clinical support staff provide well-child checks and immunizations at schools. This ensures that students have access to immunizations that are accessible and affordable.
