Our friendly providers and staff offer the expertise to help every member of the family get well and stay well! We offer a full range of primary healthcare services for patients of all ages. All services are available on an income-based sliding fee scale for patients who don’t have insurance or are under-insured. We believe no one should be denied healthcare for any reason, including financial hardship. Everyone is welcome in our clinics.

Primary & Preventive Care
Our providers can perform routine examinations and help you with all kinds of common ailments. We offer :

Flu shots are now available at all of our clinic locations. To make a flu shot appointment, call us at 612-588-9411. Please fill out a flu shot form online before your appointment. If you already have an in-clinic appointment scheduled for another reason, we encourage you to request a flu shot during that appointment.
Due to the COVID-19 outbreak, many types of appointments are now available via telehealth. This means you can meet with a provider via phone or secure video chat for appointments including:
With telehealth, you can get the care you need from providers you trust without leaving home. If your appointment requires you to be seen in person at the clinic, please wear a mask to your appointment if you have one. This will help protect our staff and other patients.
Sometimes a patient will need to be referred to a hospital or a specialist for care. Neighborhood HealthSource works with most metro-area hospitals. If a patient needs to be hospitalized, a hospital is chosen based on the patient’s specific health needs and insurance plan. If a specialist is needed, we refer patients to specialists who can accommodate similar payment arrangements to those offered at our clinics.
For delivery of a baby, Neighborhood HealthSource collaborates with
North Memorial Family Medicine. For patients who choose to have pregnancy care with us, delivery would take place at North Memorial Health or
Hennepin County Medical Center.
Chronic Disease Prevention & Management
We provide in-clinic care and at-home support to help patients live their best lives while managing chronic conditions such as diabetes, hypertension, and HIV.
Diabetes Care
Our Certified Diabetes Educator & Nutritionist offers free, one-on-one counseling at each of our clinic locations to help patients navigate life with diabetes. Patients can discuss diet and lifestyle, get tips for successful glucose testing, and access free monitoring equipment. We offer monthly group Diabetes Basics classes in both English and Spanish for patients and their families. Our Diabetes Educator also leads grocery store tours to help patients learn how to eat well and enjoy the foods they love while managing their diabetes. Community Health Workers are available to meet with patients in their homes or other community locations to work on their diabetes goals and connect them to resources for food, housing, and other needs.
We partner with several community organizations who help make healthy food available to our diabetes patients (and all patients.)
Brightside Produce Minneapolis delivers to fresh fruits and vegetables to corner stores in neighborhoods faceing food insecurity. They also offer home produce delivery. NHS diabetes patients can get 2 weeks worth of produce for free.
Open Arms Minnesota provides free meals to people with AIDS/HIV, cancer, MS, and other health conditions. They are currently also offering meals to people impacted by Covid-19.
Please note: Due to the ongoing COVID-19 outbreak in our communities, all appointments with our Certified Diabetes Educator & Nutritionist will be telehealth visits. This means that instead of coming in to the clinic for your appointment, you will meet with the provider over the phone or via HIPAA-compliant video chat. Hypertension (High Blood Pressure)
Our providers help patients with hypertension manage medication and control their blood pressure. Blood pressure monitors are available so patients can monitor their blood pressure at home. A
Community Health Worker does regular follow-up calls to make sure patients are monitoring successfully and to help them schedule in-clinic appointments as needed. Learning about and managing hypertension can help patients avoid serious health complications in the future.
HIV Care
A provider specializing in HIV care is available for patients living with HIV. We offer HIV screening during walk-in hours and at appointments. After being diagnosed, patients can schedule directly with our HIV provider, who will help manage medications and lifestyle. The provider can also connect patients to community resources for food and housing support as needed.
Sexual Health Care
We offer complete sexual health care for patients of all ages. Walk-in hours are available for low-cost or free sexually transmitted infection (STI) testing several days a week at multiple clinic locations as part of our
Seen on Da Streets program. Appointments are available at all our clinic locations for one-on-one sex education/counseling with a provider and/or health educator, as well as for birth control consults and STI treatment. Free condoms are available at all clinic sites. Teen sexual health visits are confidential.
Gender Care
A provider specializing in gender care is available for individuals who are transgender, gender-diverse or gender non-conforming. Services offered include gender hormone therapy and coordination of care with behavioral health and surgical specialists. We also provide comprehensive gender-affirming primary care, including cancer screening and management of chronic diseases.
After-Hours Care
If you have an emergency and need immediate attention, call 911 for help. If you have an urgent health problem after regular clinic hours, call us at (612) 588-9411. An operator will take your name and phone number, the patient’s name, and the reason for your call. We will return your call within the hour to advise you about what steps to take at home, whether you should seek immediate care at a clinic or hospital, or whether you can wait until the next day for an appointment at one of our clinics.
