To make an appointment at one of our clinics, call us at (612) 588-9411.

Current Neighborhood HealthSource patients can request an appointment by logging in to our patient portal.

If you have an emergency needing immediate attention, call 911 for assistance. If you have an urgent health problem after regular clinic hours, call us at (612) 588-9411. An operator will take your name and phone number, the patient’s name, and the reason for your call. We will return your call within the hour to advise you about what steps to take at home, whether you should seek immediate care at a clinic or hospital, or whether your need can wait until the next day for an appointment at one of our clinics.

Located in the Holland Neighborhood of Northeast Minneapolis.

Mission & Services

Neighborhood HealthSource provides a full range of primary medical and behavioral healthcare services at four clinics in Minneapolis and the north metro. We work hard to make sure everyone in our communities can access the care they need.

We are committed to treating all patients, with and without health insurance, and believe that cost should never be a barrier to care.

Payment Options

Providing care that our patients can afford is vital to our mission. We have an income-based sliding fee program for patients with and without insurance.
